• The client contacts our office at 506-648-6150 to receive a pre-assessment questionnaire (to be completed by their Occupational Therapist or another health-care professional, if applicable).
  
  
• Once a completed questionnaire is received by our office, the client will be added to our waitlist, and then the client will be contacted to schedule the initial assessment appointment. Typical waiting time is 3-4 months, but urgent requests will be considered.
  
  
• Upon completion of the assessment, the OT will prepare a report and funding will be requested from appropriate sources.
  
  
• Once funding for the equipment is confirmed, the equipment will be ordered by our office.

• Delivery of equipment to our facility may take up to 8 weeks.

• Once equipment arrives, it is adjusted and/or customized to meet predetermined needs.
  
  
• The client will be contacted to schedule a fitting appointment where the equipment will be adjusted to achieve an ideal fit.  The client will be provided with instructions & training on the use and care of the equipment.

• Follow-up will be provided to the client, as required, for the life of the equipment.
